GetNameservers is inconsistent across providers (#655)
* Warn if GetNameservers returns FQDN+dot strings
* Simplify logic that covers for the inconsistency
* Fix azuredns, gcloud, bind, route53
* Clean up cloudflare, digitalocean, dnsimple, gandi_v5, namedotcom
